Which Channel balance should I order?

Hello,

I need help finding the right channel balance for my Milgard window our your website. It's having trouble sliding up and hold its up position.
H-C40-56-91
H-C35*-108X90 (mull)
June 5, 2006 774336.004 1045 SUCL LOWE4V
The window width is 33inch.

To determine the correct replacement balances needed for your window, I'll need to see a photo of an undamaged balance removed from the window. Unfortunately, the sticker does not indicate the type of balance needed for your window.

I'll need to see a photo of any attachments that may be on the top or bottom of the balance and I'll need to know the stamp from the balance.

If there are no balances in the window at all, I'll need to see an undamaged balance from a similar window and I'll need to know the height of the visible glass as well as the window weight.
