Which Model of Mortise Lock do I need?

We have several patio doors with Sash Controls  model 5125703 DS3403 398 handles. All of the Mortise locks appear to be breaking down at the same time. Which Mortise lock fits into this particular Sash Control Door Handle?  I have attached a picture of the old Mortise lock but cannot find a model number. Thanks!
