Thanks for writing in! Your handle set reminds me of the 82-030 Sliding Patio Door Handle Set. It is slightly different in design but I am confident this should be the right replacement.

The internal mortise lock inside the door appears to be the 82-226 Mortise Lock. It is sold separately from the set.

We typically see damage like this to a handle set when there is damage to the roller and/or track or if there is dirt/debris lodged in them. If you're needing to apply excessive force to open and close the door, it would be advised to check the track and the rollers.
