Hello,
It looks like you have either our S351-18, with a 15-002 bottom attachment, or S390-18, with a 15-001 bottom attachment. Take a look at the image below to determine which balance you'll need.
Due to your unusual stamp code, you will need to weigh your sash to determine which option you'll need for your replacement balances. There is a chart on the product page that explains how to choose your option based on the sash weight.

The width measurement looks like 1 1/8". So it seems like we have the S390 with a 15001 bottom. Correct?

We can't make out accurate dimensions from photos but it does look like your shoe is over 1" deep. If so, then the S390 would be the way to go.
