The old part has number on it as follows: 26 4 EV CMC 2

We will be ordering several pair, once we know the correct part number to get from you. Thanks for your help

Welcome to SWISCO. Our S395-27 channel balance looks similar to your original balance. Compare your nylon fittings with ours to confirm this is the correct balance. if everything looks good, then you can go with the 2640 option.

Thanks-I took another look and believe S385-27 has the exact nylon top and bottom we need.

What is the 2640 option?

Ok, sounds good. With the way you photoed the bottom balance shoe and how it is positioned in the photos, I could not tell if it had flared wings coming out of it or not, but if looks like the Series 385, then you should be ok to go. As for the 2640, it is the stamping that corresponds yours that is marked with "26 4" which holds a sash weight of 19-26 pounds.
