Window Balance Replacements

Need help trying to find the replacements for both of these window balances. Attached are the pics.

We should be able to replace these balances, but they'll have to be specially made. Before we can begin, can you show me a clear picture of the hook at the end of the cord? Can you also tell me how many you need?
