Thank you for reaching out to us! This appears to be our Series 380 or Series 385 channel balance. The best match will depend on the bottom shoe depth as shown below. There should also be a stamp printed on the metal channel that will indicate the stamp option to select for your replacements.

Thanks for your assistance. I was able to find the stamp. 28 4 IT CMC 2

The width is 1 3/32 wide indicating series 385.

Does the 4 indicate the 2840 for weight balance?

In that case, you'd want to select the 2840 stamp option. This number represents the weight calibration. For example, a pair of S385-29 balances with the 2840 stamp will support a sash weighing 23 to 28 pounds.
