please tell me which top sash guide I should purchase see attached picture. Also, have a 30 lb window what size 34" 33 4 BQ caldwell stamped on the channel

I believe you have either our S385-34 or S380-34, depending on the size of the bottom shoe. You will need to measure your bottom shoe and refer to the dimensions we show on our site to confirm the best match.
