Window Channel Balance

I'm looking for a replacement balance that I think is Jeld Wen brand.It's 19 3/4" long and 1/2" wide. The metal hook piece that is tied to the cord is broken.

We can help replace this balance but it will have to be specially made. Before we begin, can you tell me the stamp printed on the metal channel? Can you also tell me how many you need?
