Window Crank and extension with fastener nut

Looks like the worm gear has come out of your operator. This isn't meant to be removed, unfortunately, and it only happens when the gearing in the operator is stripped out. This kind of damage can't be repaired and the worm gear itself is not something you can replace. You need an all new operator.
