A home owner from tijeras says:
The base number is 45179, the arm is 40761 and it's 12 inches. Question does it come with the "screw thing" don't know the name?

Hey, thanks for posting. I don't think your exact operator is available anymore, but our 39-348 looks close. See what you think. That "screw thing" you mentioned is the gear bore, and it comes with all operators. In fact, it's not even meant to come out. Once it has, there is no repairing it, so a new operator is the only solution.

The biggest difference between our operator and yours is the style of the base. You will need to remove your old sill cover to access the screws, but otherwise I think it could work.
