Thanks for posting! I'm happy to take a look into this to see if there is a replacement recommendation we can make but I'll need to see photos of the hardware to confirm. I'm sorry to say I am not able to identify the operator you need using the part information provided.

I'll need to see the base of the operator as well as the arm(s) and how the operator attaches to the bottom of the swinging sash.

Thanks for the photos. It appears you have an Ashland brand operator. Unfortunately, we would not be able to provide you with a replacement for the cover. While it looks similar to a few of the covers we have, ours are slightly larger and would not be a fit for your hardware.
