Thanks for writing in, Hudis. This is a Sash Finger lever, which interacts with your window's internal tilt latch. You can see our selection of them here, but frankly I have doubts you will find a replacement that works for you. The most important dimensions with these levers is the length of the prongs underneath and the distance between them, and I have a fear none in our supply will match what you have. I recommend you consider replacing your internal latches with surface-mounted tilt latches like our 26-228 and 26-229. See the video below.
