I looked at all the ones Swisco sells and can't find anything that even resembles this...any ideas?

This shoe appears to be a match for the 15-208 Pivot Lock Shoe. To determine if this is the correct replacement, remove the existing shoe and compare the dimensions. This is often used with the hook your balance has.
