Could someone please help identify the correct replacement part for the balance shoe shown in the picture.

Thanks for posting. I am confident we can help, but I am afraid there's no way of knowing what pivot shoe you need without removing it from the window and giving us its exact width and thickness. We should be able to narrow it down from there.
