Window Top Sash Guide for Channel Balance

I am asking if you could help me identify this part. It is a Top Sash Guide for Channel Balances used in my house. I have 3 windows that these have broken on, and now the Channel balances don't stay with the window sash, but now they shot to the top of the window frame. I am asking if you could help me identify this part so I can change it out, and also what would cause these to break on 3 windows, (just coincidence?) I appreciate any assistance you might be able to provide on this matter, thank you.

Thanks for posting! This looks a lot like our 18-166. Carefully compare the dimensions to your own hardware and see if you agree.
I'm not sure why they all broke at the same time. There could be a few reasons. If the windows have worked fine for a while before this, then it could be a simple matter of the top sash guides being old. If these are new windows, though, then it's possible that the balances do not have the appropriate strength spring.
