They don't seem to fit correctly

Its a kinco window.

Balance seems to be bad as well. Please advise on the right model #'s thank you for your help.

You are showing us a broken top sash guide without dimensions, so look and compare Kinco's other top sash guide, the 18-166. Compare to an undamaged guide to see if this matches up. As far as the channel balance goes, if the depth of the bottom balance fitting is 1-3/32", then you can use S385-30 with a stamp option of 2940.
