Unfortunately, I can't confirm the correct replacement for the channel balance shown in your photos since it is damaged but what I can make our resembles the S395-29. I would recommend removing an intact balance from the other side of the window to confirm that it matches up to yours. 

As for the top sash guide, take a look at the 18-124P guide part shown below and compare the dimensions to your part. Thank you!
