Window balancer for Milgard 36" single hung windows

I have 3 broken balancers in 3 different windows and they are all the same as pictured. All the windows are 36", with 3 sets of double windows in a 6' opening and 2 single windows in a 3' opening. The sash widths are slightly different at 33-5/8 for the singles and 33-7/8 for the doubles if that affects anything, but they have the same balancer. The balancer cord has a hook on one end that hooks onto the window frame. I have more pictures if needed. What is the price and availabilty for pre-assembled balancers?
These are 24 years old and all broke within a very short period of time. Is there maintenance I can perform to prevent future breakage?

I submitted a post earlier and neglected to give the dimensions of the balancers. The metal channel itself is 9/16" wide, 5/8" deep and 26-15/16" long. The balancer assembly with the plastic ends is 27-15/16" OAL. The printing on it is 26 4 AS Caldwell. Thanks.

Hello, thank you for posting,

With this combination of attachments, you could have either an S380 or S385 channel balance. You’ll need to measure as shown below. The Series 380 has a 1" bottom attachment, and the Series 385 has a 1-3/32" attachment. 

Please note that we can only supply balances on the full inch. Take a look at the 27" balance with a 2640 stamp option. 

If the balances are constantly breaking it could be that the spring is too strong. A 2640 stamp option supports a 19 to 26 lb window sash. Are you able to weigh the sash and confirm the exact weight?
