Need replacement, part number stamped on back is 31876 on arm and 012700 on body.

Looks like you have an older version of our 39-220B Encore operator. Since this is the new Encore style that Truth makes, you'll also need a 39-452 handle/cover combo. Please also see our 39-545 bracket.
