Thanks for reaching out! Your balances can be replaced with either the S378-24, S380-24, or the S385-24. To determine which of these is the correct series, you'll need to confirm the depth measurement of your bottom shoe. Please note, the 7/8" and 1" shoes are not interchangeable.

You'll need to obtain the exact sash weight once you are able to remove the sash and weigh it. It sounds like you may have a jammed window which could be resulting from a damaged top sash guide. 

That being said, I've tagged a video tutorial below detailing how to address these issues. Be sure to take a look over them to determine how to best fix the window.

I'll need to see a photo of an undamaged sash guide to identify the correct replacement. There are quite a few used with this style balance.

Thanks for the additional photos. These guides look similar to the 18-166 Top Sash Guide. Carefully look over the dimensions and compare them to confirm that this is the correct replacement.
