Please have a look at our S380-30 channel balance. This comes with a 19-005 balance top and 15-005 bottom balance shoe. Carefully compare your nylon fittings with ours to see if this is a match. since your stamping does not match with any of our options, you will need to weigh your sash on a scale and refer to the sash weight chart on the product page.
