Hi there. I believe our S395-28 channel balance will work for you. This includes the 19-005 balance top and 15-001 bottom balance shoe. See if this balance matches up with yours. I didn't notice any stampings or markings on your balance to determine the stamp option, so you may want to look for that. If there are no stampings or markings, you will need to weigh your sash on a scale and refer to the sash weight chart listed on the product page.
