Hello there. When that gear pops out along with the handle, it's a sign the entire assembly has to be replaced. We can likely help, but we need to see the entire operator, including the link arm(s) that jut out the back of it and push and pull the sash. A bird's eye view, with either the operator removed from the window or at least with the window open, would be best. From there we should be able to narrow this down quickly.

It looks like you have the 39-031 or its opposite-handed counterpart, the 39-032, depending on which side of the crank the cog assembly is on. Take a look at the diagram below. Let me know if anything differs, particularly that link arm length of 4-7/16".
