Hey Ellen. This looks like our 39-107 Cover and 39-114L handle. However, it looks like the worm gear popped out with the crank handle; when that happens, I am afraid it means the entire operator must be replaced. We can likely help; I just need to see the whole mechanism, including the link arm(s), to ID what you need.
