It's a Milgard window from about 1998.

The crank has Truth stamped on it.

Can you please give me the replacement part number?

Thanks for waiting during this busy season. I recommend you have a look at the 39-032 and 39-156. The difference between the two is the length of the link arm. Measure as shown below and see which one is the best match for your old assembly.
