Thanks for your post! The part number from your operator has been linked to two different styles of dyad cranks we have. Take a look at our 39-089 and 39-090 Window Dyad Cranks and the 39-187 and 39-188 Sash Dyad Cranks. 

To determine which style is the correct replacement for your repairs you'll need to review and compare the dimensions and the shape of the gear arm. As you'll notice on the diagrams below, the 39-089 has a "Z" shape while the 39-187 has more of "W" shape.
