Hello there. This looks a lot like either our 39-032 or 39-156 Dual Arm operators. These mechanisms are almost identical, except the link arms are different lengths. See the diagrams below for reference. Compare with your old hardware to see what matches best. In either case, for new covers and handles see the 39-114L and 39-107 sold separately.
