Thanks for reaching out to us. The top sash guide looks like the 18-069. We believe this is what you want but we recommend you review the specifications and compare them with yours to be sure.

We're happy to ID the balance but we'll need more information. Can you show us close up photos of the top and bottom fittings? Can you also show us a photo of the inside of the balance's metal channel where the innerspring connects to the pulley mechanism? That'd be located in the middle of the channel.
