Hello! Looks like you have a Kinco side-load balance system. The balance will be S380-30 (with the 19-005 top and 15-005 bottom balance fittings attached). The spring weight stamped 29-4 will be 2940. As for the top sash guide, it looks like 18-166. Please compare all these parts to be sure that they match.
