Our window sash weighs 39 lbs, it is a non-tilt window, the channel balance is 5/8" wide and 5/8" deep and the metal portion of the balance is 34" long (but the balance is stamped "33"). I can't tell from your site which series replacement to order ... possibly S351-34 with the 3365 (35-39lbs) option? Also, do I need to order the shoe and top guide separately or do they come with the sash balance. Last question ... why if it is 34" long is it stamped "33."

Hello,
It looks like you have either our S351-34, with a 15-002 bottom attachment, or S390-34, with a 15-001 bottom attachment. Take a look at the image below to determine which balance you'll need. I agree that the 3365 stamp option should work for you as long as your window sash weighs 39 pounds as indicated by the balance stamp.
The balances come with the top and bottom fittings already attached. Also, these types of balances are often 1" longer than the stamp number, which is actually equal to the height of the visible glass in the window sash. Thank you!
