Windows are failing--top sash guides to blame?

My windows in my house are failing on a daily basis now. The home is about 25 years old. I am almost certain these are Pan Am windows as I remember when the house was built there were big PA stickers on every window. There appears to be 2 modes of failure, both appear to be little plastic bits on the top of the sliding window. They are breaking and the spring channels will pop up. I have included two pictures of the different failures. Can you identify the parts I need?

Thanks for posting! I think the top sash guides you may want are our 18-042 pair, but it's hard to tell from these images. To have a better idea, I'd have to see an undamaged top sash guide removed from the window. Take a look at our 18-042 though and see what you think.
