The Worm screw has broken. Need to replace the full unit. Also need new slide that under the window
need 2 of each part

Thank you for reaching out to us. Your operator resembles the 39-032 and 39-156, depending on the length of the link arm. I'm not sure what you mean by slide, however. Could you be referring to the hinge? If so, take a look at the 36-048 to start. Review the specifications for these parts and see how they compare with your old hardware.

Note that we recommend replacing the top and bottom hinges at the same time.
