It's tough to be sure due to the condition of the hardware, but the 39-031 and 39-032 operators would be a good place to start. Review the specifications below compared to yours. If it's a match, we offer the 39-107FK handle and cover kit for both operators. These use the 39-505 track for the straight arm. I can't make out your mounting bracket, but you can find our selection here.
