Hello! This handle set looks like a match for our 82-089 Legacy handle set. Review our photos and dimensions and let me know if you agree.

We also have a keyed version, which is our 82-086.

No it has to be 4" not 3"

Yes, the hole spacing on the 82-089 and the keyed 82-086 is 3-15/16" center-to-center. This matches what you have. Let me know if you have further inquiries.
