Thanks for posting. This operator appears to match our 39-264 Andersen Split Arm Crank. I believe this is the only model. Is this what you're looking for?

Please also note, you may need a 39-362, Andersen adapter plate, it is used in conjunction with the 39-264 split-arm operator for Perma-Shield Enhanced casement window units manufactured from 1995 to 1998.
