awning operator assistance

Question

Have a Permaseal window with a mal functioning awning window operator as shown in the picture.
The width is 3/4in.
The vertical distance between the screw holes is 4.5 in.
The top hole is 5/16 in. to the left of the bottom hole.
the total height of the operator is 5 7/8 in.
the depth of the operator body is 2 1/8 in.
