It appears that you are in need of a new hinge assembly. Please start by reviewing the style and dimensions of the 36-048 2-bar hinge set that has a 10" sill track. We also have the 36-049 with a 14" sill track. Compare to your original to confirm a good fit! 

These sets come with a top and bottom hinge. They are also interchangeable on the right and left. 

Please note that both the top and bottom hinges will need to be replaced at the same time. Let us know if you have any additional questions!
