Hello there! It looks like you need a new four-bar hinge. I recommend taking a look at our selection here. Review the specifications and see if any match what you need. We just recommend you replace both hinges at the same time.

what model do I need
Part that screws into concret side is 12 inches.

If I replace both on 36-22HD Is this model for left and right?

The 36-022HD hinge is non-handed so it can be used in either left handed or right handed applications.
