-actuator arms have "TRUTH" imprinted on them, see photo 1D,

-glass has "B19191x27B2M", see photo 1E.

- need price & availability of mechanism if you can identify it?

Take a look at our 39-083 dual arm crank. This looks like the one you have. Carefully compare all measurements and dimensions to be sure and get back to us if you have any questions.
