It is an Amerirock hinge with the numbers pat 7 5027474
2010-31 on it and it is for a face mounting cabinet.

Thanks for posting. Can you tell me the diameter of the round portion that plugs into the door? That would help us find a good match.

the round portion measures 1 3/8" across
