Thanks for your post! I'm confident we have the replacements for this widow balance but I'll need to know how it attaches at the top of the window track to confirm the exact configuration. 

Does it attach using a screw or using a hook? If it attaches using a hook, these will need to be a custom assembly.

The replacement with have for the shoe is the 15-194 1" T-Lock Shoe. However, it has a closed cam in the back instead of an open cam, like yours. These are sold separately from the balances.

Thanks for confirming! You'll need the S795-24 Channel Balance with a 24C Stamp. As mentioned, the 15-194 would be the best replacement option for the shoe. Keep in mind, it does have a closed cam, which may interfere with the installation. The cam may not allow the pivot bar to install properly.
