corner cabinet hinge

I believe that I have Homecrest cabinets in my kitchen originally installed in 2003. I have a broken hinge on the corner cabinet folding door. I don't see a part number on the corner cabinet, but the drawers have the part number of HC 25653. I am having a hard time finding a replacement. Thoughts?
