crank covers missing when we purchased the house- opening measures 5 inches across- cannot locate any names or identifying marks

Looking at your photo, the operator looks like it could be something like our 39-032. Can you confirm? If I'm right, then for its cover I would recommend our 39-107. The inside measurement of the 39-107 is 4-5/8" wide.
