This window operator is the 39-253LB Encore Dual Arm Operator. The piece at the end of the link arm of your operator is the 39-529 Truth Hardware Track Slider, which slides in the 39-528 14-1/2" Casement Track.

Your cover does not appear to removable, so you'll need to purchase either the 39-375 or the 39-451. The opposite versions of the operator and the cover kits can be found tagged below as well.
