I am looking to see a photo of the base of the crank as well as the arm(s) and how the operator attaches to the bottom of the swinging window sash.

Your operator appears to be the 39-264 Andersen Split Arm Crank. To confirm that this is the right replacement for your window, carefully look over the dimensions below. If your window was manufactured between 1995 and 1998 use may also need to use the 39-362 adapter plate.
