handle crank arm falls out of the slot when opening/closing the window

I watched your video on how to replace a broken handle operator replacement. You showed in the video that there's a cut-out that allows the arm to detach from the slot bracket. My problem is the arm falls out of the cut-out when you're opening and closing the window. My slot bracket is 11 1/2". How best can I deal with the above issue so the operator handle doesn't fall out of the slot bracket when opening and closing the casement window?

Thanks for reaching out to us! The guide at the end of the operator arm definitely shouldn't be able to fall out so easily. It sounds like either the track is damaged or the guide itself. Can you show me a clear picture of both? That will give me a better idea of which one is in need of replacing. 

Please note that if the guide is broken, you will likely need an all new operator.
