I can help narrow it down. These plastic end pieces match the Series 351 or Series 390 balances. I can't tell which, though, since it comes down to nothing but the size of the shoe. If you measure like the photo below suggests, you can tell the difference. Series 351 has a 1" shoe, and Series 390 has a 1-1/8" shoe. The 2430 usually means you need a 25" balance (verify by measuring the metal bar by itself) with a 2430 stamp option (verify by weighing the window, which should weigh between 15 and 19 pounds.
Comparison of 15-001 & 15-002

The plastic ends come attached, but they vary from Series to Series, so you need to make sure that the Series you choose has plastic ends matching the balance you have at home currently.
It sounds like you figured it out, though. The Series 351 that you chose has a 1" shoe from back to front.
As far as the stamp, many customers order without weighing their sash to check the stamp, and they are usually okay. However, you're taking a small risk by trusting that the stamp is correct, even though it usually is. We do not recommend flying blind. You should weigh the window if at all possible.
