hinge for lazy susan kitchen cabinet

Question

Original hardware is Brookhaven. First photo (hardware attached to cabinet door) shows the piece I need.  Other photo shows the broken part attached to it's partner piece. The hardware  holds together the two doors (that sit at 90 deg. when closed) for a lazy susan.
