Hello!

Your operator reminds me of our 39-187 and 39-188 but compare our dimensions with yours to confirm these match up to your old assembly. I'm afraid we don't have a hinge set with a 12" base but look over the specifications for our 36-048 and 36-049. Note, you'll want to replace the top and bottom hinges at the same time.
